This is a prime example of things we see on a daily basis. This extinguisher is being held onto the bracket by its pull pin. Behind the valve assembly of the fire extinguisher, you can see the hook that is supposed to be used to attach it to the bracket. 

If the pull pin gets damaged or bent, it may not pull out properly when needed. This will cause the extinguisher to be useless.
